Congratulations again on being selected for recognition of your achievements for the 46th Annual Scholastic Achievement Foundation of Palm Beach County, Inc.’s recognition of outstanding seniors. We will be compiling a commemorative program this year and we need to know more about you. Please complete our questionnaire online at www.safpbc.org No Later Than March 1, 2024.

We would also like to include your senior photo in the program and will need you to complete the Student Photograph Consent Form and forward your photo to us with this questionnaire.

Is your outstanding high school graduation and matriculation to college unique or typical in your family. For example – first to graduate, first to go to college, first valedictorian or third, any thing else you think is interesting.

Is there anything about your family’s situation that makes this accomplishment of being selected even more special or unique. For example, immigrated recently, had to learn another language, single family home, family illness, parent also recognized by the Scholastic Achievement Foundation.

If you have been offered one or more scholarships please tell us about that: how many, how much, for what: e.g. sports, music, computers; any thing else you would like us to know? Were you a national merit semi – finalist, finalist or scholar, Pathfinder Nominee etc.
